I (parent/guardian) agree to register my young dancer (named above), as a QL2 Dance member in 2025.

I understand that membership includes two classes per week, as per the schedule and guidelines below.

I understand that membership incurs an annual payment of $1375, and that payment is due in three instalments: $475 due 31 January 2025, $475 due 30 April 2025 and $425 due 31 July 2025
